If you are a registered referee, assessor, examiner or instructor, and you did not receive a copy through the post, contact the FA via your Regional Manager to obtain a copy and to ensure that you are properly listed on the FA database.
If you are a member who is not registered with the FA or County FA, please ask your ABS Secretary to submit your name to RA Head Office and a copy will be sent to you via your ABS.
